Permission & Medical Forms!
If you are interested in participating in any of our events this year then you need to have your parents fill out these forms! Once you hand these forms in you won't need any other medical or permission slips until September of next year (unless the activity that we are participating in requires a separate form). You can print out PDF files of the forms below!
